Re: H1N1 Vaccination Clinic

Please click here for the information from the Toledo- Lucas County Health Dept. regarding the 2009 H1N1 influenza virus.

An H1N1 influenza vaccination clinic will be held for our students at the Toledo School for the Arts on Tues. Dec. 1, 2009 beginning at 8:30 AM in the 1st floor Art Gallery. We are providing the space. The Toledo- Lucas County Health Dept. will be conducting the clinic. At this clinic the vaccine will be given in shot form; no intranasal "flu mist" will be given. If your child is fearful of needles and injections you may be present to assist your child.

If your child has not received the H1N1 vaccine and you wish to have him/her vaccinated, please fill out and return the consent form by Fri. Nov. 20, 2009. This vaccine will not be given to your child without your written consent.

The mailing you will receive shortly regarding the H1N1 clinic contains only one consent form per family. If you want more than one child vaccinated in your family you will need additional
forms- one per child. (Available for download at the above link)

Please direct any question regarding the vaccine to the Toledo- Lucas County Health Dept. @ 419-213-4114. Your health care provider can also answer your questions.

Lynn Zuercher, R.N.
